NOTICE TO ALL CUSTOMERS:

ANY and ALL health issues, injuries and expenses incurred during or
after a pet’s stay at H-D Interactions Dog Training Co. are the sole
responsibility of the Pet Owner.

TERMS, CONDITIONS AND AGREEMENT FOR ALL SERVICES

This Agreement and Terms and Conditions (collectively, this “Agreement”)
constitutes a binding legal agreement between you and H-D Interactions Dog
Training Co. (“H-D”). This Agreement applies (i) to all services provided and
events sponsored by H-D, including boarding, grooming, and training and (ii)
each time you visit H-D or attend an event sponsored by H-D after the date
hereof whether or not you are asked to sign this Agreement at another future
time. You have the right to terminate this Agreement with respect to any future
visit or participation in H-D events by providing written notice of such
termination to H-D prior to such future visit participation. H-D reserves the
right to refuse service or participation to any person at any time.

VACCINATIONS

To prevent the spread of disease while your pet is with H-D, pets must be
current on Rabies, Bordetella and Distemper vaccinations. It is your
responsibility to provide proof of vaccination history for each animal that
comes on our premises. You further understand that even if a pet is vaccinated
against kennel cough (Bordetella) or other illnesses, it is possible that a pet
may contract kennel cough or another illness during its stay at H-D. You
specifically agree that H-D shall in no way be responsible for death or injury
should your pet contract kennel cough or another illness while at H-D. If your
pet is found to have fleas and/or ticks, it will be bathed and/or treated by H-D
at your expense.

MEDICAL ATTENTION

I the owner or its agent agrees to be prepared to have his or her pet
discharged from H-D within 24 hours of any given notice. If your pet requires
medical attention, H-D will attempt to contact you for instructions regarding
care. If H-D is unable to contact you within a reasonable period of time, or if
H-D otherwise determines in good faith that the condition of the pet requires
immediate medical attention, H-D is hereby authorized to administer such medical
attention as it deems appropriate, or seek medical attention for your pet from
a veterinarian designated by you or, if no such designation has been made, by
any veterinarian selected by H-D in its discretion. All costs and expenses
incurred by H-D or such veterinarian are your sole responsibility and will be
paid promptly by you.

YOUR PET'S STAY AT H-D

If your pet is left in our care for boarding, and training, H-D will provide
your pet with a clean, safe and properly maintained environment, and will
regularly feed your pet or otherwise in accordance with your reasonable
instructions. Your pet will not be removed from H-D’s premises except for “Field
Trips” or with your prior consent or in the case of emergency. Standard
precautions will be used against the injury, escape or death of pets. H-D will
not be held responsible for injuries that occur, provided that standard care and
precautions have been followed as determined at the sole discretion of H-D. You
expressly agree that H-D’s liability in the event that standard care and
precautions are not provided by H-D, shall in no event exceed the lesser of the
current chattel value of a pet of the same species or the sum of $2,500.00 (Two
Thousand Five Hundred Dollars) per animal.

RISKS INVOLVED WITH GROUP ACTIVITIES

You agree and understand that there are inherent risks when dealing with
animals and humans in group situations. Such risks include problems associated
with rough play, such as bite wounds and scratches, the aggravation of existing
medical conditions, and in rare instances death. Female pets that are in season
also run the risk of unintended pregnancy. You understand and agree that under
no circumstances shall H-D be responsible for illness, injury, or death
(including aggravation of existing injuries or conditions) to pets participating
in group activities (including training, socialization through play, or play
time out in the yard). You consent to your pet’s participation in group
activities, acknowledge the foregoing risks and limitations on liability and, if
your pet is female, certify to H-D that such pet is not in season and is not
expected to come into season during your pet’s stay at H-D.

TRAINING PROGRAMS

You agree to and understand that you are paying for and will receive the amount of lessons based on your selected program/package: 6 week Training Program - 6 one on one lessons, 8 week Training Program - 8 one on one lessons, 10 Week Training Program - 10 one on one lessons, Puppy Head Start Program - 5 one on one
sessions. These training programs/packages are
designed to educate you as the owner or agent on how to work with and train your
dog(s). These programs are non-refundable (see section labeled ‘Payment For
Services’ for more information). H-D is not held liable
for the outcome of the Training Program and no extra lessons will be added to
the Program.

PAYMENT FOR SERVICES

If registered for any board and train program, you agree to and understand
that a payment of $500 is due at time of scheduling and is non-refundable. The
remaining balance of the board and train program is due at the time that you
drop your dog off at H-D and is also non-refundable, unless discussed and agreed upon otherwise. There then will be a separate document completed and signed stating the new payment plan. All evaluations are priced at $100 and are non-refundable. If you sign up for any service at the time of the evaluation, your $100 paid for the evaluation will go towards the cost of the service. Fees for individual private lessons
are due at the time of scheduling the private lesson. If signing up for any of the Training
Programs/packages you understand that you must pay the program/package price in full at the time of scheduling unless discussed and agreed upon otherwise. There then will be a separate document completed and signed stating the new payment plan. These are non-refundable. You agree to and understand that there is a cancellation fee
of $50 should you try to cancel or reschedule any lesson without a 24-hour
notice. If a 24-hour, or more, notice is given then the lesson can be canceled
or rescheduled without the $50 fee. This can occur up to three times. If you as the client/owner/agent of the dog(s) cancel more than three times, this is grounds for discontinuation of the package/program. In order to achieve any type of result, you as the owner need to show your commitment to the training of your dog(s). Multiple cancelations of lessons shows that you are not committed. If any fees or expenses payable by you are
not paid when they are due, or if your pet is not claimed within ten days of its
scheduled pick-up date, H-D reserves the right to exercise any and all remedies
available to it at law or in equity, including, without limitation, the right to
sell your pet in accordance with applicable law to obtain these owed fees. You
agree to promptly notify H-D of any change in the scheduled pick up date of your
pet, and agree to be responsible for and pay any additional charges incurred as
a result of a change in a scheduled pick up date.
If purchasing a Personal Protection Dog (PPD) from us, you understand that a non-refundable amount of $1,000 is due to reserve that PPD for you. The remainder amount is due at the time of drop-off of the PPD to your home/location. If you do not pay the remainder amount owed in full at that time, H-D has the right to void the sale, keep the dog, and your non-refundable $1,000 deposit on the dog. You also agree to and understand that $2,500 is due on top of the original price of the dog for expenses incurred by H-D to travel to your location, any meals, or hotel fees that were needed for the three day drop-off and transition training process of the PPD. The amount owed for the PPD and drop off training must be paid by using a cashier's check. If making a payment with a
check, and the check bounces or gets returned, you as the client are responsible
for the fees incurred with that returned or bounced check, and must pay again
for the services that were provided by H-D.

RELEASE FROM LIABILITY
As additional consideration for H-D accepting your pet for any and all services,
you hereby release H-D from any and all liability (except as expressly set forth
above under the heading “Your Pet’s Stay at H-D”), cost and expense resulting
from or related to (i) loss or damage to your pet, including loss or damage
resulting from or caused by disease, running away, theft, fire, participation in
group activities or other animals or people or (ii) loss or damage to persons or
property caused or alleged to be caused by your pet. In addition, you agree to
indemnify and hold harmless H-D and its employees, agents and representatives
from any and all claims resulting from any action by your pet.

The undersigned specifically represents that he or she is the owner of the
pet(s) that are brought to H-D for services or that the undersigned has been
authorized by the owner of the pet(s) to enter into this Agreement as the duly
authorized agent of the owner. For good and valuable consideration, the receipt
and sufficiency of which are hereby acknowledged, and intending to be legally
bound hereby, H-D and the undersigned have agreed to these terms and conditions
as of the date set forth below.
